• Active medicaid and CHP+ members can log into the secure PEAKHealth app using your PEAK credentials. Create a PEAK account and check your active status at www.colorado.gov/PEAK. For additional tips, visit HealthFirstColorado.com.
• Call the PEAK help desk for help downloading, opening and using the app: 1-800-250-7741 (State relay 711), Monday-Friday, 7:30 a.m. to 5:15 p.m.

Use the app to:
• View the medical ID card for all members of your household.
• Find a doctor, dentist, pharmacy or health care facility using our map-based search.
• Know what benefits are covered by your health plan and how much you could pay.
• Update your contact information, remove a member of your household and update your job and income information.
• View your CHP+ and Medicaid Buy-in account balances. Make one-time payments for enrollment fees. Set up recurring payments for Medicaid Buy-in monthly premiums.
